Resolution: BE IT RESOLVED, that the contracts, contract amendments and administrative actions of the Human Services and Public Health Department and Department of Community Corrections and Rehabilitation made pursuant to Chapters 256E and 393 of Minnesota Statutes, as detailed in Human Services and Public Health Contract Report 1810 be approved; that the report be filed in the Contract Administration Office; that the Chair of the Board be authorized to sign the contracts, contract amendments and administrative actions on behalf of the County; and that the Controller be authorized to disburse funds as directed. Such contracts are subject to ministerial adjustment, when such adjustments are done within the constraints of the approved Hennepin County Budget and when signed by the County Department Director or Designee.

Contracted dollar estimates are based upon prior year usage and are subject to fluctuation in placement patterns, service need, and cost shifts. Background: The Housing and Urban Development s (HUD) Emergency Solutions Grant (ESG) is a formula grant program. Eligible recipients generally consist of metropolitan cities, urban counties, territories, and states. Metropolitan cities, urban counties and territories may sub grant ESG funds to private nonprofit organizations. All recipients must consult with the Continuum(s) of Care (CoC) operating within the jurisdiction in determining how to allocate ESG funds. The City of Minneapolis (the City) receives approximately 940,966 in an annual ESG grant. The City s Consolidated Plan allocates 60 of the grant to shelter rehabilitation and street outreach, 30 to rapid rehousing and homeless prevention, and 10 to administration. Like Hennepin County, the City of Minneapolis prioritizes rapid rehousing over homeless prevention. Sub-granting these ESG funds dedicated to rapid rehousing from the City to the County will: Leverage the County s expertise in social services, and Improve alignment with the County s existing rapid rehousing contracted providers and service standards. Current request: This action amends the Joint Powers Agreement with the City, which was approved by the County on July 18, 2017 (Board Resolution 17-0282) and accepted the first year of funding of 288,000 for July 1, 2017 through June 30, 2018. This amendment adds 276,136 for the period of July 1, 2018 through June 30, 2019 to the agreement for a total of 564,136.

Hennepin County will use the ESG funds from the City for rapid rehousing assistance. Rapid rehousing services resettle people experiencing homelessness quickly to permanent housing through housing

Background: In 2004, the State of Minnesota established a business plan to end long-term homelessness. In response, Hennepin County entered into a cooperative agreement with six other metro counties (Board Resolutions 06-6-358, 09-260) to develop and submit The Metropolitan Counties Long Term Homeless Supportive Services Plan to the Minnesota Department of Human Services (DHS) to fund services in supportive housing programs that would allow the seamless delivery of services and coordinated allocation of resources across metro county boundaries. Under the governance structure of this agreement, Hennepin County is designated as fiscal agent and lead agency contractor for this multi-county effort. Since the initial funding in 2006 and through December 31, 2015, DHS had continued to allocate resources totaling 31,548,589.50 towards this effort (Board Resolutions 06-6-358, 07-416, 08-323, 09-0286, 10-0280, 11- 0329R1, 12-0332, 13-0377, and 15-0193 ). The current grant was accepted by the County on December 15, 2015 (Board Resolution 15-0512). The amount of the grant was 5,734,550. Amendment 1 increased the amount by 402,821 to a new NTE of 6,137,371 (Board Resolution 16-0421). Amendment 2 extended the grant 2 years to December 31, 2019 and increased the NTE amount by 5,734,550 to a new NTE of 11,871,921 (Board Resolution 17-0414). Amendment 3 will increase the NTE amount by 180,950 to a new NTE of 12,052,871. Through Agreement A154206, Hearth Connection serves as the administrative entity for the delivery of services and allocation of resources for this initiative, and to ensure consistency in services standards and outcomes. The service dollars provided by this grant allow the metro counties to expand and coordinate the range of services to individuals and families experiencing long-term homelessness and those families at Page 14 of 17

Background: In 2017, the Minnesota Legislature passed a comprehensive housing package, adding language to the Minnesota Housing Support Act (Chapter 256I), to increase and improve opportunities for Minnesotans with disabling conditions to live in the community. The housing package included grant funding aimed at developing and supporting community living infrastructure throughout the state, totaling 2.8 million for the State through June 2019, 1.5 million for July 2019 through June 2020, and 2.6 million annually ongoing. Hennepin County HSPH convened workgroups of managers, supervisors, and planners to develop a proposal for funding, with the goals to fill key gaps in our system, leverage HSPH s existing expertise and initiatives by strongly connecting them, and develop housing expertise and resources across all of HSPH. DHS awarded Hennepin County a total of 212,842: 72,101 for a housing resource specialist, 124,268 for administration and monitoring of the Housing Support program (formerly Group Residential Housing), and 16,473 for indirect costs. Using these funds, Hennepin County HSPH will relieve the County of property tax expenditures, and fund 2 grant FTE positions for the Housing Stability Area. These resources will:
